This Notice of Privacy Practices ("Notice") describes how Lincoln Square Coaching LLC ("we", "us", or "our") may use and disclose your Protected Health Information ("PHI") to carry out treatment, payment, or health care operations, and for other purposes that are permitted or required by law. It also describes your rights to access and control your PHI. This Notice is effective as of January 1, 2024, and applies to all PHI that we maintain.

Treatment: We may use PHI to provide, coordinate, or manage your vocational rehabilitation counseling services. This may include consultation with other healthcare professionals, referral to other providers, or coordination of care with your other providers.

Payment: We may use and disclose PHI to bill and collect payment for the services we provide. This may include submitting claims to your insurance company, verifying your insurance coverage, or collecting unpaid balances.

Health Care Operations: We may use and disclose PHI for health care operations, such as quality assessment and improvement activities, staff training, and business management.

Other Uses and Disclosures: We may use or disclose your PHI without your authorization for the following purposes:

• Required by Law: We may disclose PHI to comply with federal, state, or local laws, such as reporting suspected abuse or neglect, or complying with a court order.
• Public Health Activities: We may disclose PHI for public health activities, such as reporting disease outbreaks, preventing or controlling injury, or reporting vital statistics.
• Health Oversight Activities: We may disclose PHI for health oversight activities, such as audits, investigations, inspections, or licensure.
• Judicial and Administrative Proceedings: We may disclose PHI in response to a court or administrative order, subpoena, discovery request, or other lawful process.
• Law Enforcement Purposes: We may disclose PHI to law enforcement officials for purposes such as identifying or locating a suspect, fugitive, material witness, or missing person.
• Coroners, Medical Examiners, and Funeral Directors: We may disclose PHI to coroners, medical examiners, or funeral directors to allow them to carry out their duties.
• Organ and Tissue Donation: We may disclose PHI to organizations involved in organ, eye, or tissue procurement or transplantation.
• Research: We may disclose PHI for research purposes, with your authorization or if the research has been approved by an Institutional Review Board or Privacy Board and certain conditions are met.
• Avert Serious Threat to Health or Safety: We may disclose PHI to avert a serious threat to the health or safety of you or others.
• Specialized Government Functions: We may disclose PHI for military and veterans' activities, national security and intelligence activities, protective services for the President and others, or medical suitability determinations for the U.S. Department of State.
• Workers' Compensation: We may disclose PHI to comply with workers' compensation laws and other similar programs.

Your Rights Regarding Protected Health Information

You have the following rights regarding your PHI:

• Right to Access: You have the right to inspect and obtain a copy of your PHI.
• Right to Amend: You have the right to request that we amend your PHI if you believe it is inaccurate or incomplete.
• Right to an Accounting of Disclosures: You have the right to request an accounting of disclosures of your PHI.
• Right to Request Restrictions: You have the right to request restrictions on the use or disclosure of your PHI.
• Right to Confidential Communications: You have the right to request that we communicate with you about your health information in a certain way or at a certain location.
• Right to a Paper Copy of This Notice: You have the right to a paper copy of this Notice.

Changes to This Notice

We reserve the right to change this Notice at any time. If we make changes to this Notice, we will post the revised Notice on our website and make it available to you upon request.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. There will be no retaliation for filing a complaint.
